LV non compaction A rare form of a cause of nonischemic (congenital) cardiomyopathy is LV non compaction In this disease the LV fails to βcompactβ and has extensive trabeculations with crypts and recesses throughout the LV cavity. This cardiomyopathy carries a high risk of malignant arrhythmias, thromboembolic phenomenon and left ventricular dysfunction. It is very rare with a frequency of 0.05%. Patients will often present later in life with heart failure or an embolic event. Diagnosis is made via contrast echo (usually in PSAX) measuring the noncompacted Lv to compacted LV with a ratio >2 or MRI with a ratio in end diastole of >2.3 Treatment consists of goal directed medical therapy for heart failure. Controversy exists with the use of anticoagulation as well as ICD placement.
