However, I must note that the tone and assumptions expressed in your recent messages are not appropriate for professional scientific communication. To avoid any misunderstanding, I confirm what I previously communicated: the manuscript “Neurostimulation over craniovertebral region: an emerging therapy” (Manuscript ID: 1698387) is currently not suitable for publication as a Perspective Article in its present form. This assessment is not personal. It is based on scientific and editorial reasons, including an article-type mismatch and overstated claims. Although the manuscript is submitted as a Perspective Article, it reads largely as a hybrid narrative review and position paper, with extended didactic sections (anatomy, reflexes, disorders) and conclusions that imply clinical effectiveness. A Perspective must clearly distinguish what is established evidence from what is proposed as hypothesis, and should not present speculative mechanisms as if they were confirmed therapeutic effects. The statements are too definitive for a Perspective, because the manuscript does not provide direct clinical evidence supporting such broad therapeutic claims for the proposed intervention, and the mechanistic assertions are presented with a level of certainty that is not justified by the cited literature. Additionally, the “Cervical neurostimulation” section combines different modalities (e.g., cervical spinal cord stimulation, transcutaneous spinal cord stimulation, occipital nerve stimulation, and muscle stimulation) without sufficiently separating their mechanisms and evidence levels.
